Certificate of Registration of Establishment / Facilities / Business / Workspaces
In accordance with the provisions of the Occupational Safety and Health (Registration of Premises, Facilities, Enterprises and Workplaces) Regulations 2019, a "Registration Certificate" is issued for premises, facilities, Enterprises and / or workplaces for which employers or self-employed persons apply for sponsorship from any Ministry or have applied for registration in a register or directory of a Ministry or other State Agency.
In order to obtain the above Certificate, the employer or self-employed person must complete the relevant DLI-APL-1 Application Form and send it to the relevant District Labour Inspection Office in the District where the premises, facility, enterprise or the workplace is located.
This Application must be accompanied by the following documents:
1. Architectural plan or outline of the premises or facility or workplace in A4 or A3 size drawing sheet showing the current situation and clearly showing the name of the premises or facility or workplace, the emergency exits and the sign of the direction in which they can be opened, sanitary facilities, the kitchen area, and any storage areas of flammable and / or petroleum products (liquids and liquefied petroleum gas) or other chemicals.
2. (a) A copy of the Certificate of Establishment of a Legal Entity from the Department of Registrar of Companies and Official Receiver, if the applicant is a Legal Entity, or
(b) A copy of the applicant's identification card if the applicant is an individual
3. A copy of the Certificate of the Registrar of Companies and Official Receiver, showing the shareholders or directors of the Company (where applicable).
4. Employer’s Liability Insurance Certificate for all employees (where applicable).
5. Rental document of the premises or facility or workplace, (where applicable).
6. Planning Permit or Building Permit or Certificate of Approval or Lease Agreement in the case of a state-owned land, indicating the approved use of the premises or facility or workplace for which the application is being submitted.
7. Documents / evidence that a Written Risk Assessment (WRA) has been prepared and is available at the premises or facility or workplace for which the application is being submitted (e.g. person's information who prepared the WRA, date of preparation or date of last update of WRA). The WRA must be available at the premises or facility or workplace for which an application is being submitted for onsite inspection by the Competent Labour Inspector.
8. Documents supporting the implementation of a Risk Management System [business organizational structure, details of persons involved in protection and prevention activities, safety committee function, training data / registers (online e-learning system - egnosis), accident report registers, emergency procedures, list of risk management procedures and work instructions or Certificate of the use of ISO 45001, (where applicable)].
9. Copy of receipt of the specified fee issued by the Accounting office of the Department of Labour Inspection. For the examination of the application, the fixed fee of €220 must be paid for issuance or €175 for reissue of the Certificate of Registration. The payment of the fee can be made in cash or by bank check or by postal check at the Accounting Office, at the Head Office of the Department of Labour Inspection, which will issue a relevant receipt. The check, which must be issued to the "Director of the Department of Labour Inspection", can be delivered by hand or sent by post.
